Rehabilitation & Community

Stroke rehabilitation helps people with stroke-related impairments regain their abilities and independence. It includes: hospital-based care (acute and post-acute), outpatient programs and community support.

The goal is to improve physical, cognitive, emotional, and social functions. After initial rehabilitation, community services help stroke survivors and families adjust to their new lifestyle.

“Recovery is a process. It takes time. It takes patience. It takes everything you've got.”

“I am proud to have been a part of this project. When I had my stroke, I found it hard to focus. A map of where I was that showed my stroke journey and the recovery process including what lay ahead would have been useful. I am optimistic that other stroke survivors find hope for their future from this document.”
