The Northwestern Ontario Regional Stroke Network is a vital component of the Ontario Stroke System with a commitment to Northwestern Ontario residents. Serving as one of 11 regional networks dedicated to enhancing stroke care across the province.

Our mission encompasses:

Evidence-Based Practice Integration: We implement and promote best practices to enhance outcomes for stroke survivors and their families.

Optimized Care Delivery: Our focus is on continuously improving and streamlining stroke care delivery systems throughout the region.

Comprehensive Care Continuum: We work collaboratively across all stages of stroke care, from prevention to rehabilitation and community reintegration.

System-Level Improvements: We actively promote and facilitate systemic changes to advance stroke care quality and efficiency.

Professional Development: We provide and advocate for high-quality educational opportunities for healthcare professionals involved in stroke care.

Public Awareness: We support and develop initiatives to increase public awareness about stroke prevention, recognition, and management.

Through these strategic efforts, we strive to maintain and improve stroke care standards in Northwestern Ontario, ensuring our region keeps pace with provincial benchmarks in key performance indicators.

Northwestern Ontario Regional Stroke Network Team

Regional Director/Manager

Plans, implements and evaluates all operations of the Northwestern Ontario Regional Stroke Network. Responsible for coordinating a system of stroke care that is evidence-based, integrated, comprehensive, and covers the full continuum of care.

Medical Lead

Works in partnership with the Manager to lead the Northwestern Ontario Regional Stroke Network. Provides medical leadership to ensure high standards of care for people with stroke.

Regional Stroke Clinical Nurse Specialist

Co-ordinates and facilitates learning opportunities related to stroke care throughout the continuum of stroke care from prevention, acute care, rehabilitation to community re-engagement including long-term care.

Stroke Community Engagement Specialist

Promotes and supports stroke best practice and knowledge in awareness, prevention and pre-hospital areas of the stroke continuum. With a focus on stroke system optimization, the role collaborates with health care providers, care facilities and organizations across the region in primary care, emergency medical services and hospitals.

Regional Stroke Rehabilitation Specialist

Promotes and supports stroke best practices across the rehabilitation and community continuum. Collaborates with rehabilitation and community providers, facilities and agencies across the region to address rehabilitation and community needs

Administrative Assistant

Supports the manager and team with office operations.

Meetings/Committees

The NWORSN Meetings and Committees provide guidance and bring a regional voice to stroke care.

Stroke Transitions Group

In alignment with Canadian Stroke Best Practice Recommendations, and in the context of health system transformation in Ontario, the overarching purpose of the Stroke Transitions Group is to work as a collective partnership to ensure that patients with stroke and their families experience smooth and efficient transitions between care settings, and are supported to successfully re-engage in the community.

NWORSN Community Stroke Prevention Clinic Meeting

The NWORSN Community Stroke Prevention Clinic meeting supports stroke care and stroke services for secondary stroke prevention and with a smaller focus with primary stroke prevention as it relates to people at high risk for stroke, recurrent stroke, in Northwestern Ontario.

Regional Primary Care Meeting

The Regional Primary Care meetings will support the professionals working in primary care supporting stroke patients in 3 domains: 1. High risk patients; 2. Acute stroke/TIA patients and 3. Post-stroke patients. The virtual meetings will be hosted by the NWORSN team and held bi-annually with focus on best practices, education and learning to support the providers that work within Northwestern Ontario.
